The same families who leave nothing to chance in business, health, and wealth often have a young adult drifting toward dependency instead of independence. You've tried treatment, therapists, ultimatums, second chances. What you haven't had is a system.
I work with the family, not just the young adult, because that's where the pattern actually lives. Private. Evidence based. Handled with complete discretion.
If you have built real success in your life, you already know how to solve hard problems. This one has not responded to the usual tools. Not to effort, not to resources, not to the best help money can find. That is the part almost no one says out loud.
Sound familiar?
01
Are you losing sleep over your young adult's impulsive decisions, risky behavior, or constant defiance?
02
Are you worried about their mental health, or their struggles with vaping, cannabis, alcohol, gaming, or a screen they never put down?
03
Are you watching them stall out, with no job, no plan, and no drive, wondering if they will ever build the confidence to launch into adulthood?
You're not alone, and you don't have to do this alone.

With 24 years of clinical experience and thousands of families, Thomas Miller helps parents transform the family system, not just the young adult, so struggling 18 to 30 year olds move from dependency toward independence.
He became a licensed clinical social worker in 2002 after earning his Master's in Social Work from NYU, and has led at nearly every level of care. He co-founded a large group therapy practice, served as Family Wellness Director at an elite treatment center for adults with co-occurring disorders, was Clinical Director of a residential therapeutic program serving adolescents from throughout the world, and directed both the Center of Hope and the Den for Grieving Kids.
It is a noisy world. What sets this work apart is a strength-based, solution-focused approach grounded in deep clinical experience, and a huge heart. He works directly and privately with a limited number of families at a time.
Most families arrive having already tried everything. Treatment. Therapists. Ultimatums. Second chances. What they have not had is a system, a sequenced process that changes the family, not just the young adult.
The 4 Peaks Parents Program is that system. It is the compendium of 24 years of clinical work with thousands of families across every level of care. Each peak builds on the one before it. Nothing is theoretical. Every stage produces visible movement.
Before anything changes, the system gets examined. Parents map the family dynamic, the rules, roles, and patterns operating beneath the surface, many of them invisible, many of them unintentional, and many of them holding the young adult exactly where they are. You finish this peak with a clear read on what is actually driving the pattern.
Every parent knows what they should do. Almost none can hold it under pressure. This peak builds the steadiness required to hold your ground through the guilt, the pushback, and the escalation that show up the moment things shift. This is where confidence replaces secondguessing.
New skills for new terrain. Precise language, clear responses, and the real
mechanics of boundaries, understood correctly as commitments about your own actions, not rules imposed on your young adult. Financial structure, communication scripts, response protocols. By this peak, you know exactly what to say and do in the moments that used to derail you.
The old patterns are exhausted and the new ones become the culture. Families establish the standards and operating agreements that will define the next generation, and the young adult begins moving because the environment around them has changed. This peak turns a temporary shift into a permanent family identity.
Watching, applying, and being coached privately, at the same time. That is what separates families who understand the method from families who execute it.
23 videos that walk you through the full method step by step, available to you immediately.
A 75-page working document that turns each concept into applied work on your family's actual situation, so progress is visible and I can meet you where you are stuck.
Direct, private guidance with me, for one or both parents, as the patterns shift and the pushback arrives. When the moment is right, your young adult joins the conversation, and I facilitate the honest dialogue that helps your family hear each other and start communicating in a new way

Next-Gen Readiness is the work of preparing the next generation to carry what they inherit. Not only the money, but the responsibility, the independence, and the character that money cannot create on its own.
Most families invest heavily in the transfer of wealth and almost nothing in the readiness of the person receiving it. That gap is where capable young adults quietly become dependent ones.
This is the work that closes it.

Many of the families I work with are visible in their communities, their industries, or their professions. For them, privacy is not a preference. It is the condition that makes asking for help possible at all.
Every engagement is confidential. Sessions are scheduled privately. Nothing is shared, published, or used as an example without your explicit permission, and many of the families I have helped will never appear anywhere on this website.
In-depth assessment of current dynamics.
Custom behavioral roadmap development.
Guided implementation and coaching.
Long-term integration and support.

No job, no plan, no momentum. The adult child living at home, dependent, with no clear path forward. This is the core of the work.

Whether it's mental health, addiction, neurodivergent challenges, or all of the above, the house now runs on their mood. Everyone else is walking on eggshells, managing around the next outburst or the next crisis instead of living their own lives.

You want to help without harming, to set boundaries without cutting them off. This is how you stop enabling and start building readiness.

This is the core of what I do. The 4 Peaks Parents Program works specifically with families whose young adult — typically 18 to 30 — has stalled out completely. The starting point is never the young adult. It's the family system around them, because that system is almost always what's keeping them exactly where they are.
It's real — and laziness is almost never the actual cause. In my 24 years of clinical work, the young adults who appear unmotivated are almost always contending with underlying anxiety, depression, substance use, or executive function challenges. The behavior looks like laziness. The driver is almost always something else.
Most often a combination of underlying mental health challenges — anxiety, depression, ADHD — and a family dynamic that has, with the best intentions, made staying stuck more comfortable than moving forward. Removing natural consequences, absorbing responsibilities, and accommodating avoidance are the most common contributing patterns. All of them are modifiable.
In my experience, duration is less about the young adult and more about when the family system changes. Families who complete the 4 Peaks Parents Program — 90 days, fully committed — consistently see meaningful movement. Young adults who have been stuck for years, sometimes a decade, begin to move when the environment around them finally shifts.
It is increasingly common — but common is not the same as acceptable, and it is not inevitable. A 25-year-old with no plan, no momentum, and no accountability is not on a trajectory that fixes itself without intervention. The question is not whether this is normal. The question is whether you are ready to change it.
Yes — consistently. Anxiety and depression are the most common underlying drivers of stalled independence. They show up as avoidance, withdrawal, and apparent lack of motivation, and they frequently get misread as laziness or defiance. The 4 Peaks Parents Program addresses the family dynamic that has formed around these conditions, because that dynamic often determines whether a young adult stays stuck or begins moving.
Most likely both — and neither label changes what needs to happen. Cannabis use at that level almost always coexists with underlying anxiety or depression, and the family's response to it — tolerating it, accommodating it, funding it indirectly — is usually part of what sustains it. I work with the family system around this pattern, not just the behavior itself.
This is one of the most common situations I work with. Treatment gains rarely hold when a young adult returns to an unchanged family system. The 4 Peaks Parents Program addresses precisely this gap — building the family's capacity to hold new expectations, maintain new boundaries, and stop the accommodation patterns that treatment alone cannot reach.
ADHD and executive function challenges make the tasks of daily independence — scheduling, follow-through, self-management — genuinely harder. My work helps parents distinguish between what is outside their young adult's control and what is within it, so they can support the first without excusing the second. That distinction is where the work begins.
The clearest signal is a pattern of avoidance that has organized the entire family around it — everyone managing, accommodating, or walking on eggshells. Mental health challenges rarely exist in isolation from family dynamics. I help parents develop a clear picture of what is actually driving the pattern, because that clarity is the prerequisite for everything else.
Almost certainly — and almost certainly with the best intentions. The most common enabling patterns are absorbing your young adult's responsibilities, removing natural consequences, funding avoidance, and managing their emotional state to keep the peace. None of these feel like enabling from the inside. The Family Audit in Peak One of my program identifies exactly which patterns are operating in your family.
This is the central tension most families bring to me — and it has a precise answer. Enabling ends not by withdrawing love but by withdrawing accommodation. The 4 Peaks Parents Program teaches parents to set boundaries as commitments about their own actions — not rules imposed on their young adult — which allows structure and connection to coexist.
No. The patterns that keep a young adult stuck almost always come from love and concern, not poor parenting, and they are common even in the most capable, most resourced families. What matters now is that these patterns are identifiable and modifiable. That is the starting point of the work — not a verdict on what came before.
This is more common than most families admit — and parental disagreement is one of the most powerful factors sustaining a young adult's stuck pattern, because it creates inconsistency the young adult navigates rather than accountability they have to face. The 4 Peaks Parents Program works with one or both parents and addresses alignment as a core part of the process.
The key is understanding what a boundary actually is. A boundary is not a rule you impose on your young adult. It is a commitment about your own actions — what you will and will not do going forward. That reframe changes everything. Boundaries set this way protect the relationship rather than threatening it, because they are about your behavior, not theirs.
Parent coaching focuses on the parents — their patterns, their communication, their role in the family dynamic — rather than on treating the young adult directly. Therapy for the young adult addresses their internal experience. Parent coaching changes the external environment around them. In my experience, changing that environment is what most families have not yet tried — and what most consistently produces results.
Yes — and this is precisely how the 4 Peaks Parents Program is designed. Your young adult's willingness to participate is not a requirement to begin. Change in the family's own patterns is the starting point, and in my experience it is what typically precedes any shift in the young adult. You do not need their permission to change the dynamic.
Thomas Miller Coaching works specifically with high-net-worth, high-achieving families whose meticulous approach to career and wealth has not extended to preparing a struggling young adult for independence. With 24 years of clinical experience across every level of behavioral health care, this practice addresses the specific gap that exists in accomplished families — what I call Next-Gen Readiness.
The 4 Peaks Parents Program is a structured 90-day private engagement: 24 individual sessions with me, immediate access to a 23-module video library, and a 75-page workbook your family works through in real time. It follows four sequential stages — Family Audit, Emotional Mastery, Upgrading the Toolkit, and Rewriting Your Family Story. Nothing is open-ended. Every stage produces visible movement.
Most families begin noticing shifts within the first few weeks — not because the young adult has changed, but because the parents have. The dynamic feels different. The conversations go differently. The old patterns have less room to operate. Meaningful, sustainable movement in the young adult typically follows within 60 to 90 days of consistent work.
