You do not need another opinion on what your young adult should do. You need a process. The 4 Peaks method is the same step-by-step approach Thomas has used with families for more than 20 years, to move you from crisis to clarity, one peak at a time.
Most families who come to Thomas have already done the work - or what looked like the work. Therapy. Treatment programs. Interventions. Honest conversations that went nowhere. They have tried harder, given more, and waited longer than anyone should have to.
What they have not done is change the system.
That is where the Four Peaks framework begins. Not with the young adult. With the family - because the family is not the cause of the problem. It is the most powerful lever for solving it. The Four Peaks is a sequential, 90-day framework built on a single conviction: when the family system changes, everything else can.
"Year after year, I watch the same thing play out. The families with the most capacity to help are often the ones who have the hardest time not shielding their young adult from the experiences that build competence, resilience, and independence. That is the irony nobody names."
— Thomas Miller

Each peak builds on the one before it. You cannot rebuild trust before you have learned to respond differently, and you cannot launch a young adult before that trust holds. The climb is sequential, and every stage produces visible movement.
What actually happens at each peak, and what you walk away with.
Before anything can change, you have to see clearly what is actually happening-not the story you've been telling, but the real dynamics, roles, and unspoken agreements organized around your young adult's struggle.
Identify exactly what the family system is producing and why.
Locate key leverage points within the family dynamic.
Shift focus from managing the problem to becoming the primary change agent.
A clear view of the real underlying patterns.
The foundational truth that the family system is the change agent.
New conditions that make moving forward more comfortable than staying stuck.
Your young adult’s anxiety, anger, or withdrawal has been quietly driving household decisions. Peak Two focuses on mastering your emotional state so their storm no longer determines your response
Break the pattern of young adult emotional outbursts dictating household rules.
Learn to remain regulated, calm, grounded, and clear in high-friction moments.
Lead your family from a place of stability rather than chaos.
Reliable emotional self-regulation under pressure.
The capacity to co-regulate and stabilize your young adult.
A solid foundation to lead your family with confidence.
Insight without skill produces frustration. Peak Three introduces practical, evidence-based tools tailored specifically to your family's dynamic
Develop heart-centered communication strategies that open doors.
Establish flexible, timely, and purposeful boundaries built from intention.
Calmly shift the weight of responsibility back onto your young adult.
Practical, real-world communication tools.
Boundaries established without exhaustion or fighting.
Restored sense of agency and purpose for your young adult.
Peak Four is deliberate, structured preparation for what comes next-anticipating setbacks and ensuring the changes made during the 90 days become permanent.
Play the tape forward to anticipate setbacks and handle potential relapses.
Build a resilient family culture strong enough to weather future storms.
Replace old holding patterns with a system that continuously sustains growth.
A launched, resilient young adult moving forward independently.
A long-term sustainable family operating system.
A new family narrative that lasts well beyond the 90 days.
The framework is not a menu. Each peak builds on the one before it. You cannot master your emotions before you understand the system producing them. You cannot upgrade your toolkit before you are regulated enough to use it. You cannot rewrite your family story before you have the skills to live it. This is why the sequence matters. And why 90 days - done with precision and full commitment - changes what years of conventional approaches could not

With more than 20 years of clinical experience, Thomas Miller helps parents transform the family system, not just the young adult, so struggling 18 to 30 year olds move from dependency toward independence.
He became a licensed clinical social worker in 2002 after earning his Master's in Social Work from NYU, and has led at nearly every level of care. He co-founded an outpatient therapy practice, served as Family Wellness Director at an elite treatment center for adults with co-occurring disorders, and directed both the Center of Hope and the Den for Grieving Kids.
It is a noisy world. What sets this work apart is a strength-based, solution-focused approach grounded in deep clinical experience and a huge heart. He works directly and privately with a limited number of families at a time.
The 4 Peaks is the roadmap. This is the operating rhythm underneath it, the same four beats every engagement.
A full assessment of where your family stands today, the patterns underneath, and what has actually been keeping your young adult stuck.
A custom roadmap built for your family and your situation, not a generic approach applied to you.
Twice-weekly coaching sessions, real accountability, and course corrections as reality intervenes.
Long-term check-ins to make sure the progress holds after the core engagement, so the change becomes permanent.
Watching, applying, and being coached privately, at the same time. That is what separates families who understand the method from families who execute it.
A step-by-step walkthrough of the full method, mapped to each peak. Available immediately, so you can watch, revisit, and work at your own pace
A live document that turns each concept into applied work on your family's actual situation. Progress is tracked in real time, so I can see where you are stuck and meet you there.
Direct guidance with Thomas throughout the engagement, so the plan adjusts as your family's situation changes and the pushback arrives.
All four, in sequence. Each peak depends on the one before it. You cannot hold steady under pressure before you understand the pattern, and you cannot rewrite the family story before the new tools are in place. The climb works because it is sequential.
No. This work starts with you. In most families, the parent's shift is what eventually creates the young adult's. Your young adult does not need to be willing yet.
Therapy treats the individual. This work changes the family system, with you as the leverage point. Most families I meet have already tried therapy for their young adult. This is different. It changes what you do, precisely enough that your young adult has room to move. My clinical training informs all of it, but the engagement is coaching.
The core engagement runs 90 days, with twice-weekly sessions. Most families feel a shift within the first few weeks. Long-term check-ins keep the change in place after that.
Schedule a private discovery call to discuss your family's situation and explore whether this is the right fit. If it is not, I will tell you that too.
