There is a particular kind of exhaustion that comes with being an exceptional parent of a struggling young adult. It is not the exhaustion of not trying-it is the exhaustion of trying everything and watching none of it work. Thomas Miller works directly with the family system around the young adult to stop enabling, shift dynamics, and create real movement.

Thomas Miller did not arrive at this work through a textbook. He built his expertise across more than two decades of leadership in some of the most demanding corners of behavioral health-wilderness therapy, elite residential treatment, outpatient clinical care, and private practice
He co-founded a comprehensive mental health group practice, served as Family Wellness Director at Mountainside (an elite treatment center for co-occurring addiction and mental health), and has built and led clinical programs at virtually every level of care. He has sat in rooms where families were at their most desperate, their most defensive, and finally, their most honest.
"In my experience, the most powerful lever for change is never the person who is stuck. It is the people who love them most and have been, without knowing it, making it easier to stay that way."
— Thomas Miller
Accomplished & High-Functioning: Parents who have built exceptional careers and lives, but are watching their young adult fail to build any life at all.
Privately Exhausted: Families who have already tried what the system recommended-the therapists, the programs, the endless conversations-and are secretly wondering if anything will work.
Ready for Systemic Change: Parents willing to examine the family system surrounding the young adult to break enabling patterns and raise expectations.

Thomas's specialty is helping families launch a struggling young adult. Behind that focus is a full clinical background that most parent coaches simply do not have.
His areas of clinical expertise includes:

Alcohol, cannabis, and other substances, codependency, and the way addiction moves through an entire family.

Anxiety, depression, OCD, trauma and PTSD, stress, and anger management

Autism, ADHD, oppositional defiance, and learning and school challenges.

Family conflict, divorce and other transitions, adoption, identity, and the college and early-career years where so many young adults stall.
Thomas earned his Master's in Social Work from NYU and has been a Licensed Clinical Social Worker since 2002. His work draws on family systems, attachment-based therapy, CBT, DBT, and motivational interviewing. What he brings to every family is precision, directness, and a proven framework-not a generic playbook or passive listening.

Many of the families I work with are visible in their communities, their industries, or their professions. For them, privacy is not a preference. It is the condition that makes asking for help possible at all.
Every engagement is confidential. Sessions are scheduled privately. Nothing is shared, published, or used as an example without your explicit permission, and many of the families I have helped will never appear anywhere on this website.
From Families
Thomas and the 4 Peaks Program gave us straightforward truths and invaluable insights which helped us tweak our family culture and communications. We weren't sure where to turn for help at a scary and confusing time. We will be forever grateful that we found Thomas Miller and this program. I cannot recommend it highly enough. We will reference and use our new insights often!
With his keen insight and abundant resourcefulness, Thomas guided us in making sound decisions on the path to our son's recovery.
Thomas - having your guidance made all the difference. You truly got us through one of the worst times. I personally do not know what I would have done without you.
Thomas not only guided us through a very turbulent time with our daughter, he also helped us prioritize our marriage and get our whole family back on track.
Thomas came into our lives at a pivotal time. When our son started to struggle with anxiety & OCD we again enlisted his help. He genuinely invested in helping our whole family.
Three things, working together from day one. Immediate access to the 4 Peaks video library, a step-by-step walkthrough of the full method. The 4 Peaks workbook, a live document that turns each concept into applied work on your family's actual situation. And weekly individual sessions with one or both parents, direct guidance in real time as the patterns shift and the pushback arrives.

No. That is the most common situation I see, and it is not a barrier. This work starts with you. In most families, the parent's shift is what eventually creates the young adult's.
That is exactly what a private consultation is for. We will talk through your family's situation and I will give you an honest read on the right starting point, or tell you if I am not the right fit.
Thomas works with a small number of families at a time and engagements are selective. Let's gain clarity, reset, and move forward together.
